Welcome to the Equitable Learning Services (ELS) online registration form.

This is the first step in connecting with ELS and obtaining educational adjustments for your studies.

Please note that as part of the registration, you will be required to provide supporting medical documentation.

If you can’t provide documentation now, please download this Health Professional Report and ask your treating practitioner to fill it out. (As the student you need to fully complete Section A on page 1, and your practitioner needs to complete the rest).

I acknowledge and agree that in order for the Equitable Learning Services (ELS) team to arrange support:

all relevant student details and subject information held in University's student systems, may be accessed by your Equitable Learning Facilitator (ELF).
my support plan may be shared within the ELS team and relevant university staff on a need to know basis (such as your Faculty course Co-ordinator, Exams team)
Relevant information may be shared with a dedicated support person, if one is required.
